We often utilize subcontractors
for a range of services including earthwork, concrete, structural
steel fabrication/erection, roofing, interior finishes, plumbing,
electrical, and HVAC tasks.
LCS takes pride in its commitment
to safety and quality of work and we expect our subcontractors
to follow suit.
We require the completion of the attached Prequalification
Form and Insurance Certificates.
A non-binding summary of our
insurance requirements follows:
- Workers’ Compensation insurance, including
employer’s
liability insurance with a minimum liability limit of $500,000
per accident.
- Comprehensive General Liability insurance policy
in a combined single limit amount of not less than $1,000,000.
- Comprehensive automobile liability insurance with
a limit of not less than $1,000,000.
- Contractor and/or its designees shall be named as
additional insured parties on all policies of insurance required.
- Subcontractor shall promptly furnish certificates
of insurance in duplicate to the Contractor’s office,
but in all events within five (5) days from the date of execution
hereof. Such certificate of insurance shall state that such
insurance coverage shall be non-cancelable except on 30 days’ prior
written notice by the insurance carrier to Contractor.
